With the new year, we would like to make goals that make a difference. Make a difference for our patients, caregivers and family members. We believe in excellence in everything we do. When you volunteer for us, we train you and bring you through an orientation. Next we walk you through simple steps of how to assist our patients by simply making a phone call.
This tuck in call is simple. You ask them if they need any supplies before the weekend, or are they running out of things? It communicates care and support, and it also helps us keep supplies going to the right place.

Make a difference. Come into our office once a week or twice a month to make calls. We will assist you and help with the training.

If you are interested please contact me. I would love to hear from you!

1913 W. Tacoma St, Suite G & H, Broken Arrow, OK 74012, US

Mission Statement

Our goal is to deliver high quality treatment to our patients, families and caregivers with our highly trained hospice team. The patient experience is at the core of everything we do. That’s why we work collaboratively with the patient’s team of experts to craft a plan of care that meets their specific needs. Our skilled nurses, physical therapists, occupational therapists, speech language pathologists, medical social workers and home health aides use a coordinated, inter-professional approach to deliver compassionate, specialized care in the comfort of home.

Description

Caring for individuals and navigating them through the end seasons of life. We compassionately care through our team touching their lives in every aspect of their lives.
Through our Community Care Program, we are able to provide a better way to care to patients living in assisted or independent living facilities, as well as residential care communities. We take a coordinated approach to care that allows patients to safely age in place.
