Recruiting Organization: Comprehensive Care, Community, and Culture (C4P) AmeriCorps Program

AmeriCorps & C4P We are now accepting applications for positions for the 2021-22 year. We have positions that will begin throughout the late summer and fall and starting as late as January 2022.

Below are the position descriptions for positions at the University of Chicago Medical Center and Ingalls Memorial Hospital.

What is AmeriCorps?

AmeriCorps is a national service program that involves people in "getting things done" in communities. AmeriCorps members develop an ethic of service while strengthening local communities. Each year in Illinois, thousands of AmeriCorps members give millions of dollars in service back to their communities. For up-to-date information on the impact on our state, visit

Serve Illinois administers 45 AmeriCorps State programs in operation for the 2021-22 program year and over 1,800 Members committing to a year of service. These Members serve 70+ counties in over 400 host sites.

What is C4P?

The Comprehensive Care, Community and Culture Program (C4P) aims to improve the overall health and well-being of patients in the program by addressing a wide range of medical and social needs. The C4P program builds off of the Comprehensive Care Physician Program, which offers patients at high risk of hospitalization a comprehensive care physician (CCP) who cares for them in both the hospital and the clinic. In addition to receiving care from a CCP, C4P patients are regularly asked about a range of unmet social needs and given access to a community health worker and community-based arts and culture programming. The Robert Wood Johnson Foundation has funded a randomized trial of C4P at the University of Chicago.

The CCP and C4P Programs are currently being expanded to and studied within new health care settings, including community hospitals like Ingalls Memorial Hospital.

The Comprehensive Care, Community and Culture Program (C4P) is proud to be an Illinois State AmeriCorps program. C4P will host up to 17 full time members across all of its participating service sites in 2021-22.

Where will I serve as a C4P AmeriCorps member?

We are currently accepting applications for two sites:

  • University of Chicago Medical Center (Hyde Park campus). A car is not required to serve at this site.
  • Ingalls Memorial Hospital in Harvey, Illinois. A car is required to serve at this site.

Applicants may apply to both sites but they must submit a separate application to both application portals (see below for more information).

Members based at Ingalls Memorial Hospital will spend 1-2 months at the University of Chicago for training at the beginning of their service and typically come to UChicago for service activities 1-2 days per week during the service term.

What will I do as an AmeriCorps member with C4P?

AmeriCorps members will be responsible for extending the reach of the C4P program to impact patient experience with care and health outcomes, lower hospitalization rates, and resolve unmet social needs for participants. Program activities may vary based on programmatic needs and the needs of the service site.

  1. Recruit Medicare patients of diverse ages who are at increased risk of hospitalization for the C4P program, which is designed to serve their medical and social needs

  3. Leverage volunteers to help recruit participants and evaluate the C4P program

How is the C4P AmeriCorps member experience impacted by COVID-19?

Our members are currently engaging in teleservice due to the COVID-19 pandemic. We expect that new members who join us during the 2021-22 academic year may have some training activities remote; however, we anticipate that members will make a full transition to in-person service beginning late summer or fall 2021. We will also follow organization and local guidelines, and we ask members to be flexible in the event that we have to return to teleservice at some point during the year.

How do I apply?

To apply to positions at both University of Chicago and Ingalls, you must submit an application through our website.

You will be asked to submit:

  1. A resume
  2. A cover letter or statement describing your interest in the C4P AmeriCorps Program

Select candidates will be invited to interview over the phone or in person.
