When a COR youth or young adult participant is housed, the host home will be matched with a Host Home Liaison, who is the host home’s front-line support person. Host Home Liaisons can help source needed items for host home or participant, connect host homes with community resources, and connect host homes with one another [with prior consent]. For the first six months of a new host home relationship, a Host Home Liaison and Young Adult Support will arrange a home visit once every six weeks [no more than 4 visits] with host family and participant. The purpose of the check-ins will be to review the signed agreement and change as needed, to identify needs and ways to meet/respond to those needs and, to continue to ensure the relationship is functioning for all involved. The Host Home Liaison will help mediate conversations between host home and participant if needed.
The Host Home Liaison is not clinical staff and cannot offer crisis management or support for a host home. They can provide support and problem-solving assistance, including referrals to other services and professionals who may be better equipped to help in the circumstances.
Additional responsibilities of Host Home Liaisons may involve:
- Checking in with potential host homes to check on potential interest and to help match them with participants needing a home
- Helping to facilitate monthly meetings of all host homes. The purpose of the meeting is to create a community among host homes, offer ongoing training, and address any needs or concerns.
- Participating in COR info sessions or trainings
This Host Home Liaison is a volunteer with Choosing Our Roots and will have: a signed Community Investor Consent to Serve, a signed Code of Conduct, and have completed the Introductory Skill share in full.
