Do you bring joy to others through your homemade gifts? Do you like to craft?

At Continuum Care Hospice, we have a unique volunteer opportunity just for you! As a part of our bereavement program, we provide families with a Forever Bear keepsake to remember their loved one. Our Forever Bears are sewn by our wonderful team of volunteers and can be made from the comfort of your own home! Materials and patterns for the bears will be provided as well as specialized hospice training, at no cost!

Our Volunteers have an immeasurable impact on the team at Continuum Care Hospice and the families we care for. With the help of Volunteers, families who receive Forever Bears are reminded that their loved one is never forgotten.

The mission of Continuum Care Hospice is to provide comprehensive coordinated care for persons with life-limiting illness so they may live as fully and comfortably as possible until the end of life and offer contiued support to the bereaved.

Description

The goal of hospice is to provide palliative care that addresses the physical, psychosocial, emotional, and spiritual dimensions of service for the terminally ill as an integrated whole. Continuum Care Hospice is guided by the principals of responsive care, teamwork, leadership and stewardship. Care to the patient and caregivers will enable those served to experience death with grace and dignity.
