At Continuum Care Hospice and Affinity Health Management, we provide terminally ill patients and their loved ones with the support and care they require to live their lives comfortably. Our goal is to provide physical, emotional, and spiritual comfort for the patient and provide tools, information and support for the patient's family so that they feel confident in the crucial decisions that come along with terminal illnesses. Description

Affinity offers hospice care to individuals with terminal illnesses in Virginia. Family members, who have not faced this situation may ask "what is hospice care?" Hospice care focuses on providing terminally ill patients with end-of-life support services that improve their quality of life and allow them to pass with dignity and respect. Our hospice care provides trained staff members who are on call 24/7 and work with the patient and their family develop a care plan that supports their physical, emotional, and spiritual wellbeing. As part of Affinity's hospice care, your loved one receives: Pain and symptom management, home health care services, training on how to care for your loved one at home, access to social services, music therapy and short-term inpatient care
