Haven Hospice Community Resources team focuses on providing support to individuals and their families who are coping with advanced illness. We are currently looking for Transitions volunteers to provide telephone support, community resource referrals, and education for people who are dealing with chronic and life limiting illness who are not ready for hospice care.
