This position will provide direct services to clients, to include but not limited to, intake, assessment and case management, and client advocacy (resources and referrals) to homeless and low-income individuals, with the goal of moving clients from crisis to self-sufficiency and to support the mission of Interfaith Community Services.

  • Provides direct services to clients including diversion, Individual Service Plan (ISP), assessment, crisis intervention, advocacy, and case management
  • Provides guidance and support to help clients identify their own strengths, set achievable goals and follow through consistently so they can become self-sufficient
  • Utilizes Housing First and Diversion strategies to stabilize clients experiencing or at-risk of experiencing homelessness
  • Administers supportive services per agency and contract guidelines
  • Identifies resources and advocates for clients’ needs
  • Links and networks clients with resources within the agency and utilizes other public and private social service agencies as necessary
  • Conducts street outreach and engagement with individuals experiencing homelessness
  • Develop and maintain effective relationships with mental health providers, SUD treatment providers as well as additional county and community nonprofit based self-sufficiency services to ensure that clients with ongoing barriers and treatment needs are successfully transferred to longer term services
  • Participates in community-based outreach events for the purpose of engagement and client linkage
  • Identify, engage and link underserved populations with mental health and substance use treatment through street outreach services

  • 2021-2022 School Year
  • • 2nd year level Master’s Degree student in Social Services or related field; or equivalent combination of education and experience required • 1+ years of relevant work experience required • Bilingual (Spanish/English) preferred
