Moments Hospice is a Medicare Certified Hospice Agency serving most of Minnesota and Wisconsin and Des Moines Iowa, Chicago Illinois and Sioux Falls, South Dakota. We are focused on creating individual plans of care, because every patient and family is unique. Moments Hospice is committed to honoring patient’s and family’s wishes during the end of life process. We take great pride in creating long lasting partnerships with our Hospitals, Facilities and Community Partners by recognizing that we are guests in their facilities and that collaborating on a joint plan of care always results in the best outcome for the patient. It is our mission to help increase the communities awareness of the Hospice Benefit as we believe that everyone deserves to be comfortable with an optimal quality of life during the end of life process.
Moments Hospice is looking for compassionate volunteers willing to donate their time to spend quality time with our hospice patients and their families, doing PET THERAPY who are in private homes, assisted livings or skilled nursing facilities in Minnesota and Wisconsin. Our hospice volunteers fill vital roles that range from direct contact with our patients and families. On any given day, our hospice volunteers may be out spending time with patients and families. We may be able to help you with the cost of certification.

- At least four hours a month for a minimum of 6 months commitment. We may be able to help you with the cost of certification.
- This position requires the volunteer to be a US Citizen or Permanent Resident so we can do a background check with a Social Security number or Green Card number.
