PHP developers - Symptom reporting site to detect pandemics

The WoIDMo is a NGO that monitors events related to infectious diseases. We are looking for a couple of seasoned PHP developers who are interested in helping to set up a website for reporting of symptoms.
Pandemics are frequent. There is flu and TB which are persistent pandemics but others are less predictable and sometimes difficult to spot. We are looking to develop more tools to detect unusual events. One of these tools is reporting of symptoms when people feel ill. For this we want to develop a site on the domain: reportapandemic.com.
This project will be open-source. Credits can be put at the bottom of the site, along with a link to yours.

Mission Statement
The World Infectious Disease Monitoring Organization (WoIDMo) is a non-profit organization, established in 2020, that brings together a network of individuals from around the world. Our mission is to raise awareness of the importance of monitoring infectious diseases globally and how they affect us. One of the important aspects of our foundation is to function as an early warning system for emerging diseases. We also support development of better diagnostics and enable fast delivery of diagnostics where needed, especially where resource-limited countries are involved. For this we work together with other NGO’s that have boots on the ground. To further our understanding of infectious diseases and how to deal with them, we aim to support researchers working on projects relevant to our goals.

We are a non-political organization who's aim is to raise awareness and make a difference in ways that are as practical as possible. We can use support in multiple area's like copywriting, social media, marketing, webdesign and web app programming.
