During these times of social distancing, our independant seniors in Yountivlle, St. Helena and Calistoga need our medically necessary transportation more than ever.
Do you like to drive? The Up Valley Village Manager will supply you with personal protective equipment and infection control training to make your vehicle safe before and after the trip!
These appointments happen during the weekday and are usually 2-4 hours in duration.

Mission Statement
The staff and volunteers of Collabria Care are committed to providing exceptional professional health care, expert resources and compassionate support to individuals and their families experiencing the transitions of aging, learning to live with chronic illness or facing the end of life. Collabria Care has been dedicated to serving the needs of Napa Valley for close to four decades. Formerly Napa Valley Hospice and Adult Day Care our new name reflects the continuum of care that we provide for those experiencing the transitions of aging. We are an Alzheimer's Resource Center, Palliative Care Provider, Information and Assistance, Honoring Choices Napa Valley in addition to providing outstanding hospice care and recently developing the Up Valley Village which is a membership program for those older adults striving to stay independent. Volunteers are utilized in all of these programs.
