Borland Free Clinic is a faith-based clinic located on the campus of Rolling Hills Community Church in Tualatin, Oregon. We provide free primary care to adults and children who are uninsured or underinsured.
Position summary: We are looking for a certified Diabetes Prevention Lifestyle Coach to volunteer to teach a Spanish-speaking cohort as part of our CDC-recognized Diabetes Prevention program.
What You'll Do
Schedule and complete telephonic DPP coaching sessions with Spanish-speaking members using CDC-certified and approved curriculum designed for the 12-month program.
Help members understand prediabetes, how it affects risk for type 2 diabetes, and how they can manage it with lifestyle changes.
Employ confidence-promoting techniques in patient communication and develop patient self-efficacy to better manage health.
Use behavior change techniques to help members develop healthy habits.
Assist/support members in safely setting and achieving goals such as losing weight, increasing physical activity levels, and making healthier food choices to lower risk for developing diabetes.
Support members with customer support, enrollment, device ordering, and translations for app use.
Document member progress and results.
Contribute to open team forums on challenges and process improvements by listening, sharing input and ideas, and enacting decisions and plans.
Maintain confidentiality of all regulated information in compliance with state and federal laws, including but not limited to Protected Health Information (PHI) and Personal Identifiable Identification (PII).
What You'll Need
Bilingual English/Spanish required
Experience in healthcare, diabetes prevention and/or management, or a related field
Strong written and verbal communication skills with exceptional listening abilities
Passion for improving people's health and happiness
Ability to motivate others
Compassionate, kind, and open-minded
Strong organizational and problem-solving skills
Ability to work independently and as part of a team in a startup environment
Education and Experience
National DPP lifestyle coach or Master Trainer certification through a CDC-approved organization
2+ years of experience in health education and/or coaching for disease prevention, behavior change, and/or health promotion is preferred

Founded in 2012 we are a faith-based free Primary Care Clinic. In 2017 we began offering a CDC-recognized Diabetes Prevention Program and classes for patients struggling to control their diabetes. In 2018 we opened our doors to anyone who does not have insurance or is underinsured seeking medical care. We are run almost entirely by volunteers and do not charge for any of our services. We are the only free medical clinic in our area seeing both adults and children. We are growing rapidly as we seek to provide services to an increasing patient population of Latino, homeless, immigrant and other underserved communities. We offer assistance to those seeking eligibility for Oregon Health Plan and wanting help in locating social services and free medications.
