Veterans Honoring Veterans at the End of Life
One in four hospice patient in our country is a Veteran who served in various wars: WWII, Korean Conflict, Vietnam War, Gulf War, Iraqi Freedom. Some have served our military during peacetime.
As a Veteran, you can honor our hospice Veterans by sharing your stories and by providing a special tribute with a pinning ceremony for their dedication and service to our armed forces.

To provide quality care for patients facing a life limiting illness and to support their loved ones. Our vision is to exceed the expectations of patients and their families by providing outstanding care, comfort and dignity through physical, emotional and spiritual support.
Description
Bridge Hospice provides comfort and support compassionately to patients and their families when a life limiting illness no longer responds to cure-oriented treatment. Our interdisciplinary approach is designed to manage all symptoms related to end stage illnesses, including pain, wound and respiratory management, while attending to the patient's physical, psychosocial and spiritual needs as well. We currently serve hospice patients throughout Alameda, San Mateo, Santa Clara and Stanislaus Counties. Our offices are located in Fremont and Modesto, California.
