The coordinator will oversee the program by identifying client needs through frequent communication and coordinating the fulfillment of those needs by contacting JCCM’s various partners, staff, and volunteers. Duties include scheduling doctor/nurse visits and Care Manager contact and follow up, and ensuring shelter, food, and medication needs are met. Medical background is desired. We are looking for someone with patience, compassion, availability, organization with attention to detail, calmness under stress, a sense of humor, and ability to work with at-risk, vulnerable clients. This is an evolving program, and the coordinator will help develop policies and procedures to make this a sustainable, successful program.
