Jefferson County Community Ministries (JCCM) has recently instituted a Medical Respite program which provides care and safe shelter for those clients who were recently released from the hospital and those extremely ill including those affected by COVID-19. The coordinator will oversee the program by identifying client needs through frequent communication and coordinating the fulfillment of those needs by contacting JCCM’s various partners, staff, and volunteers. Duties include scheduling doctor/nurse visits and Care Manager contact and follow up, and ensuring shelter, food, and medication needs are met. Medical background is desired. We are looking for someone with patience, compassion, availability, organization with attention to detail, calmness under stress, a sense of humor, and ability to work with at-risk, vulnerable clients. This is an evolving program, and the coordinator will help develop policies and procedures to make this a sustainable, successful program.

Description
For years, JCCM has devoted attention and resources to short-term emergency assistance including food, shelter and clothing. Currently, we are moving towards activities that will help clients find more permanent solutions.
A variety of life skill opportunities are in place to help with the move beyond emergency to solution in the areas of budget and financial planning, food budgeting and planning, problem solving, time management, and pre-employment activities. Over time, we will continue to add more services and programs to support our clients.
