Hearts & Minds Activity Center is a nonprofit organization located in San Jose, CA. We are a dementia-specific adult day program that serves individuals with dementia across all stages of the disease. Since COVID-19, we have moved our in-person day services to a virtual format via Zoom.
How can you help? We are organizing a virtual volunteer holiday concert via Zoom for our seniors. Each volunteer would have a few minutes to perform a holiday song of their choosing for our group. This event will be held on Thursday, December 17th from 3:30- 4:30pm Pacific time. Please take note of the time if you are in a different time zone. If you are interested in participating, please let me know the following information: 1) Your name, age, and city/state you're located in2) What your musical talent is (i.e. what instrument you play)3) What holiday song you'd like to perform
Experience (specifically with playing music) over Zoom is helpful.

Description
Founded in 1984, Hearts & Minds Activity Center emerged as the first large capacity dementia-specific adult day program in the country. Since its inception, Hearts & Minds has provided services to over 6,000 individuals living with dementia and their families. Our adult day program offers the following:
Over 60 weekly hours of caregiver relief
Daily intergenerational activities
Variety of activities tailored to individual’s impairment level
Active peer and multigenerational socialization
Secure outdoor space designed for individuals with dementia
Shower, podiatry, and haircut services
Ongoing community education and support
Respite services offered six days per week, Monday through Friday from 8:30 AM - 5:00 PM
