At Hospice of the Midwest , our primary ambition is to bring first class care to the warmest, most serene environment possible: home. We provide the full spectrum of hospice treatment, thoughtfully tailored to patients and families, with focused attention given to individual needs and preferences. Hospice of the Midwest integrates Volunteers into the hospice program by actively recruiting, training, supporting, and utilizing Volunteers to meet the needs of the patient and family. Volunteers are a highly respected and valued member of the Patient Care Team.

Description

Hospice of the Midwest provides hospice services to patient's with a terminal illness and a life expectancy of six (6) months or less.

Hospice services are available 24 hours a day, 7 days a week, (including Holidays), if needed, and are provided in the comfort of the patient's home, ("home" may be a skilled nursing facility, group home, assisted living complex, or a private residence).

A team of professionals, including a Physician, Nurse, Social Worker, Chaplain, Certified Nursing Assistant (CNA), and a Volunteer, is assigned to each patient, and will provide individualized care based on the needs of the patient and family.
