About American Hospice and Home Health Care Services
6850 Regional St. Ste 100A, Dublin, CA 94568, US
American Hospice and Home Health Care Services' mission is to provide a compassionate, quality, end of life and grief support to patients and their families. We seek to provide emotional comfort to both the patient and their family/caregiver during the last phase of life.
American Hospice and Home Health Care (AHHHC) implements programs and services to provide comprehensive, quality home health care in the patient/client’s home. We address the patient/client's unique psychological, safety, self-esteem, and self-actualization needs in a client plan of home health care.
We serve as a resource for the client, the client's family, and the community. We provide health care education for the client, the client's family, and the community. These services are provided by Registered Nurses, Licensed Vocational Nurses, Physical, Occupational and Speech Therapists, Medical Social Workers, and Home Health Aides.
AHHHC services Alameda, Contra Costa, Solano County, San Joaquin and Stanislaus counties. We continue to adhere to standards set forth by Federal, State, Local and Accrediting Bodies.
