Haven Hospice Community Resources team focuses on providing support to individuals and their families who are coping with advanced illness. We are currently looking for volunteers who are interested in training to be Advanced Care Planning Facilitators to assist people of all ages, especially Senior Citizens, with having conversations about current and future goals of care, education , identifying healthcare agents and completing advanced directives. Training will be provided.

We also need Volunteers for Transitions, Speakers Bureau and Administrative assistance.
