ORGANIZATION: FSL Helping You Live Independently

The Care By Design program promotes coordination of care throughout FSL to meet the complete health and social service needs of clients, their families and caregivers. Volunteers within the program will support the efforts of the Care By Design staff, with an ultimate goal of optimizing client outcomes and relieving caregiver stress.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Makes telephone calls to existing enrolled clients to introduce FSL and identify additional needs
  • Follows assigned script and questionnaire to determine level of follow-up required
  • Provides basic resource information utilizing the Community Resource Guide to community members who call in seeking assistance
  • Inputs data into Apricot and Excel databases
  • Assembles informational packets of FSL programs and services
  • Strictly adheres to HIPPA guidelines regarding patient confidentiality
  • Updates Community Resource Guide by calling local agencies for current service/program description

QUALIFICATIONS (including knowledge, skills, abilities, work experience, & education):

Minimum Required:

  • Bilingual in English and Spanish
  • Must be able to pass a background check
  • Comfortable with data entry (database training will be provided)
  • Is willing to commit to a minimum of a 3-hour shift, preferably 2 times per week
  • Good documentation skills
  • Possess good written, verbal, and organizational skills
  • Able to work well with other team members

Preferred:

High school diploma or equivalent and a desire in helping with the care needs of elderly and disabled persons

For more than 45 years, FSL has been caring for Arizonans. Our mission is to provide home and community-based services and develop energy-efficient, affordable housing to promote health, independence, and dignity for all. We do this by providing:

  • Safe Home: We develop both single- and multi-family homes, and we manage multi-family affordable housing complexes and residential behavioral Health Therapeutic Home Programs.
  • Nutritious Meals: We provide 375,000 pounds of food to families in need from our two food pantries in the rural Arizona towns of Wickenberg and Williams. Additionally, we serve nearly 800 congregate and home-delivered meals to Arizonans each day.
  • Health & Wellness: We provide home and community-based services to enhance independence for Arizonans of all ages, including adult day health, Medicare-certified home health, and other non-medical services.
  • Education: We educate clients, family and professional caregivers, and members of the community in an effort to empower them to maintain and enhance independence for themselves, their loved ones, and their clients.
