Join an Innovative Program that:

  • improves the health of our community.
  • is a collaboration between multiple professionals at Valley Medical Center (physicians, nurses, Registered Dietitian, Social Worker, and administrators)

  • allows you the opportunity to provide one-on-one encouragement and coaching to a patient as well as monthly rounding with a team of primary care professionals.

  • provides you with weekly professional development and team learning opportunities throughout the duration of the experience.

  • teaches valuable communication skills, including an introduction to motivational interviewing, and offers first-hand experience working with patients.

  • offers a one-of-a-kind community service opportunity that will look great on your resume when applying for future positions in healthcare.

Through this program, volunteers will obtain education related to chronic medical conditions and insights for promoting positive health behaviors. Applicants who successfully complete the 12-week online course will then receive 6 months of real-world training and experience helping patients make positive health and lifestyle changes through Valley Medical Center’s Primary Care Clinics. Health Coaches provide extended, one-on-one coaching and support in patient homes, community locations and clinic settings and virtual meetings via Zoom.

REQUIREMENTS

  • Background Check
  • Must be at least 18
  • Orientation or Training
  • Fulfillment of the 12 week virtual course (once a week for 2 hours), followed by a 6 month experiential learning opportunity during which you will visit with your patient, research resources, and attend team meetings for a total 2-4 hours per week.
  • Professional experience is NOT required. Adults over 18 are welcome. Applicants must meet our onboarding requirements. Applicants will be screened through an interview process. A cover letter and resume or summary of experience must be submitted.
