POSITION PROFILE: The Volunteer Leader will be integral in helping build and expand the Health & Wellness Program for our residents. We want our residents to be safe and happy while exercising, engaged on their health and wellness journey, and actively pursuing good health.
DUTIES AND RESPONSIBILITIES:
· Attend a weekly Health Class (Wednesdays 10am-11am) to meet Clients and assist them in Goal Setting based on their Health Assessment
· Assist Clients with health insurance navigation, medical care navigation, and setting up appointments
· Regularly meet and follow-up with Clients to ensure they are going to the doctor and meeting their health goals
· Assist Clients in obtaining and arranging transportation to and from doctors’ appointments utilizing public transportation and other public health resources.
· Work closely with Case Managers to ensure Clients are reaching their goals
· Create and implement a tracking system for Clients’ stages of reaching their Health Goals
QUALIFICATIONS:
· Education and Experience:
o Post-High School education, preferred
o Previous experience with logistics, communication, and/or social work is encouraged
o Microsoft word proficiency, internet proficiency and some knowledge in professional development
o Very well-organize
o Previous experience in Healthcare, encouraged
· Skill Requirements:
o Ability to communicate effectively in English, Spanish is also helpful
o Demonstrate maturity and sensitivity to issues of faith, culture, and other sources of diversity;
o Ability to plan and organize work and attend to detail
o Ability to handle job-related matters in a professional and diplomatic manner
· Physical Requirements:
o Hearing and speaking ability which allows for effective oral communication of information
o Physical, emotional and spiritual stamina to handle job-related issues and stress
· Other:
o Demonstrates a passion for the mission and vision of Metropolitan Ministries
