30% of elderly patients who discharge from a hospital readmit within 3 months, most often because they did not or could not meet their follow up needs. Beyond Blue is a non-medical demand response transportation program that uses volunteers to help senior clients who reside within McKinney's 75069 zip code. Support activities can include driving clients to: medical and non-medical appointments, grocery store visits, social activities, or work during days, evenings or weekends with suitable notice. Program Coordinator will provide volunteer training and client support services. Volunteers are on a roster that is notified of a specific need. Volunteers use their own vehicle and have the option of participating during times and days that are suitable for their schedule.
