Continuum Care Hospice needs volunteer for our Petaluma office to do weekly check-in calls with our patients. These volunteers play a vital role in helping bridge the gap between our patients and our Hospice Team. Our patients enjoy and appreciate a phone call checking in weekly to help provide non-invasive support and a sense of community for them. It also helps our hospice team better take care of them as the volunteers help make sure they have enough supplies and medication for the weekend. These volunteers can also help by tuning the Hospice team into issues or symptoms early on so the patient's issue can be resolved faster. It greatly helps our patients and staff to have these volunteers and allows a person to give back to their community without having to make in person visits. We need reliable, organized, and compassionate volunteers to help us with our "Tuck-In Calls." We will provide you with continual training, scripts, and support.
- Understanding and supportive of hospice philosophy.
- Compassionate person who is accepting and respectful of people's differences.
- Must be a licensed driver with an insured automobile in good working order.
- The ability to communicate well, both verbally and in writing.
- Must be able to pass background check requirements.
- CMS requires that all patient-facing staff, as well as those that are in contact with patient-facing staff, be fully vaccinated, or have a documented religious or medical exemption. This important rule is required for all healthcare organizations that receive federal funds through Medicare or Medicaid.
