Based on pre-determined criteria and supporting documentation the volunteer will determine if the applicant qualifies for the insulin voucher program. It will be the responsibility of the volunteer to communicate the decision to the applicant and coordinate efforts to ensure the pharmacy has access to the prescription. Throughout the process the volunteer will communicate to both the applicant and to the pharmacy to ensure the patient is provided insulin in a timely manner. Follow-up with the patient will be required. The volunteer will also be responsible for tracking information provided by applicants in an excel spreadsheet.
May work from home and/or at United Way office. Approximately 10 hours a week that may be divided amongst 1 to 3 volunteers (so, 3 to 5 hours per volunteer).
