Mission Statement
Heartland Hospice provides quality care to patients with a limited life expectancy, as well as support to their families and caregivers. Formerly known as ProMedica Hospice, Heartland enhances the quality of life of those we serve through palliative and specialized supportive care designed to meet the physical, psychosocial, and spiritual needs of the patients and caregivers. Hospice care is provided by using an interdisciplinary approach in delivering care to patients--either in their own homes or extended care facilities--while working cooperatively with other health care providers and other community resources. Heartland Hospice strives to provide care to all who are eligible and need hospice care irrespective of ability to pay. Heartland is part of Gentiva Health Services.
Description
Heartland Hospice serves patients in Washtenaw, Livingston, Wayne, Oakland, Lenawee, Jackson and Ingham Counties in Michigan.
Hospice supports, cares for and comforts patients in the final stages of life, as well helping as their families. Heartland serves patients wherever they call home, whether it is a private home or a nursing home or another facility. Hospice includes palliative care by nurses, as well as counseling for patient and family, and pastoral or spiritual support.
Hospice volunteers help patients and families by providing companionship, respite care and emotional or spiritual support, depending on the patient's needs.
