1.Time Commitment
15 or more hours per week on weekdays 8:30am-5:00pm, 3 month minimum
2.Job summary:
The primary objective of this internship is to support NorthPoint Community Health Workers (CHW’s) to expand communications and outreach to Spanish-speaking seniors age 65+ who are living in their own homes to provide them with resources to continue to live healthy lives.
3.Desired qualifications:
* Comfort working with seniors ages 65+ and their families
* Spanish/English bilingual fluency
* Interest in communications and community engagement
* Excellent customer service skills with positive and friendly attitude
* Experience and proficiency in Microsoft Office, Excel Spreadsheets and PowerPoint presentations as well as willingness to learn how to use Apricot database
* Good problem solving skills
4.Job Responsibilities
Support the Live Well at Home Program team (Project Manager and Community Health Workers) with community engagement with Spanish-speaking patients. The volunteer/intern will:
* Support research and provide recommendations for engaging Spanish-speaking community members to sign up for this program
* Support research and provide recommendations for using NorthPoint’s social media to engage the community
* Participate with home visits with the CHWs and provide interpreting as needed
* Assist Project Manager with tracking patients and other administrative tasks as assigned by staff

Description
NorthPoint Health & Wellness Center is a comprehensive health and human services agency located in the heart of North Minneapolis. NorthPoint is comprised of two organizations: a community services nonprofit with a broad array of social and human services (NorthPoint Inc.); and a full service community health center (NorthPoint Health & Wellness Center) that includes medical, dental and behavioral health clinics.
