ORGANIZATION: Human Health Project

HHP is seeking a Peer Navigator for the Patient Advocacy Program . In this role, you will interact with patients and caregivers concerning their difficulties navigating the healthcare system. The goal of the program is to provide peer-to-peer information, education, and support. HHP will provide training, guidance, and support for this volunteer position.

Duties & Responsibilities

  • Provide emotional support to alleviate fears of and barriers to accessing quality health care
  • Assist assigned clients to identify and progress toward achievement of their chosen goals
  • Motivate and educate individuals/families/caregivers about the importance of preventative services, medical conditions, and recovery strategies
  • Empower clients in meeting their goals
  • Provide face-to-face contact with each client assigned when possible
  • Advocate for and assist clients and their families in all areas of treatment, including accessing and receiving services such as medical care, housing, food and clothing as indicated
  • Develop individual service plans for each client with their direct input
  • Coach clients in developing age-appropriate social skills, money management, personal hygiene, and job readiness
  • Interact productively within the team in order to provide holistic mental health and case management services to an identified group of clients
  • Provide "on-the-spot" counseling that is both helpful to the clients and consistent with the philosophy of the program
  • Attend to the safety, health, and wellbeing of clients
  • Complete case management records and notes as assigned in a timely manner
  • Adhere to and uphold the policies and procedures of Human Health Project
  • Attend all team and staff meetings
  • Help bring awareness and support for HHP's programs

Essential Skills

  • Bachelor’s degree in a social service/mental health related field and two years’ experience in that field is desired but not essential
  • "Lived" experience is a plus
  • Meets all state required conditions of employment as set forth by Community Care Licensing for a licensed Community Care Facility; i.e. Fingerprint and Child Abuse Index Clearance, TB/Health Physical
  • Reliable means of transportation capable of passing a vehicle safety inspection
  • Knowledge of psychosocial rehabilitation treatment and programming
  • Ability to work and communicate with staff, clients, families, community agencies and professionals
  • Ability to perform crisis intervention strategies
  • Work effectively under stress and conflict
  • Exercise appropriate judgment and decision-making
  • Flexible and adaptable in any given situation
  • Ability to work with minimal supervision
  • Articulate, professional, credible, & ethical
  • Compassionate and caring manner
  • Excellent writing and verbal communication skills
  • Effective under the pressure of deadlines
  • Attention to detail and extremely accurate
  • Experience within the healthcare industry
  • Strong rapport building skills and the ability to develop relationships at all levels including patients, patient groups and other health organizations
  • Highly self-motivated
  • Team player with a flexible approach
  • Motivated and passionate about HHP’s mission
  • Self-starter, reliable, honest, dependable and flexible
  • Bi-lingual in English and Spanish preferred
