WISE & Healthy Aging, a multi-service, nonprofit, community-based organization has been awarded an innovative and exciting 3-year training and intervention grant. Objectives of the project are to: 1) develop and deliver supportive services to persons living alone with dementia; 2) improve the quality and effectiveness of programs and services dedicated to individuals with intellectual/developmental disabilities and dementia or at high risk of dementia; and 3) deliver behavioral symptom management training and expert consultation to family caregivers. This is a temporary, grant-based program.

The Volunteer Casual Caregiver (VCC) is a critical member of the Capability Care Project team engaged in Objective 1 above, having the most frequent contact with program clients. VCCs serve as the "eye and "Ears" of the program via their weekly, friendly visits with assigned clients , provide companionship and oversight regarding the functioning concerns to be reported back to the assigned Care Manager for actionable interventions.

VCCs attend all required training modules prior to embarking on volunteer visits. Weekly in-home visitation, complete the Volunteer Home Visit Checklist at each client contact; participate in on-going continuing education and training and weekly Supervision with the Project Manager and other staff/volunteers to ensure care plans are being followed and appropriate support is being provided to both volunteers and clients alike.

We serve three groups of people -- the frail elderly, caregivers of older adults, and active older adults age 50 and older (Boomers). We have the sole distinction in Los Angeles of serving seniors in both the traditional, residential community and those who are residents in long-term care facilities.

With our main office in Santa Monica, we also have regional offices for our City & County of Los Angeles Long-Term Care Ombudsman Program and Elder Abuse Prevention Program in Van Nuys, Pasadena, Montebello and Long Beach. For our Club WISE Program, we have centers in Santa Monica and Baldwin Hills. Programming focuses on keeping older adults age 50 and older active and healthy, with lifelong learning and volunteer engagement opportunities. Volunteer Club WISE instructors teach courses on fitness, health, technology, arts and humanities. Our Intergenerational Tutoring Program has volunteers paired with students in elementary schools in Central L.A. and also in the San Fernando Valley.
