Support Full Life Care’s Health Home program, which provides no-cost, long-term services for individuals living with chronic conditions and at high risk for hospitalization. Individuals are paired with a care coordinator to identify and achieve personal goals.
Primary Responsibilities:
- Compile client welcome packets and keep these and other standard pieces in stock for Health Home care coordinator and outreach staff.
- Organize and send letters to clients, including welcome and continued outreach letter mailings and disenrollment notices.
- Manage the start to end mailing process, including address mail merge, level and letter printing, envelope stuffing, completing postage, and documentation of mailings.
- Process scanning and standard naming of client file documents into electronic storage system.
Additional Responsibilities (Dependent on education, training, and experience):
- Answer main Health Home phone line and directing messages as appropriate.
- Outreach to clients via phone.
Skills:
- Experience or interest in social services and community health.
- Strong organizational and communication skills.
- Proficient with Microsoft Office, basic experience with mail merge and Excel preferred.
