Support Full Life Care’s Health Home program, which provides no-cost, long-term services for individuals living with chronic conditions and at high risk for hospitalization. Individuals are paired with a care coordinator to identify and achieve personal goals.

Primary Responsibilities:

  1. Outreach to clients via phone.
  2. Search within available systems for accurate contact information in the event of non-working phone numbers.
  3. Document all contact attempts in Health Home database and Excel tracking spreadsheet.
  4. Schedule initial visits between outreach team or care coordinators with interested enrollees and complete necessary related documentation and calendaring.
  5. Complete opt-out paperwork, as applicable.
  6. Mail Health Home informational material to interested enrollees, as applicable.

Additional Responsibilities (Dependent on education, training, and experience):

  1. Answer main Health Home phone line and direct messages as appropriate.
  2. Assist with appointment rescheduling on behalf of outreach team, care coordinators, or enrollees/clients as needed.
  3. Scan client file documents into electronic storage system as needed.
  4. Process closed case files; check for required documents, complete closed file checklist, and filing.

Skills:

  1. Experience or interest in social services and community health.
  2. Strong organizational and communication skills.
  3. Strong problem-solving and multi-tasking skills.
  4. Proficient with Microsoft Office, basic experience with mail merge and Excel preferred.
  5. Front desk or reception experience preferred.

  • Background Check
  • Must be at least 18
  • Orientation or Training
  • Consistent shift(s) for 4 – 5+ hours / week for at least 3 months, during Open Hours (Monday – Friday, 8:30 – 5:00). Exact times negotiable, based on volunteer’s personal schedule and office’s needs.
