The case manager conducts assessments of clients and their families. Based on the needs identified in an assessment, the case manager then arranges, coordinates and monitors multiple services from different providers to serve client needs. These providers can include state and local social service agencies and nonprofit organizations. The best candidate for this position will be acquainted with the service providers in Hot Springs and Garland County. The ultimate goal for case management at CCMC is to help the client find their way to self-sufficiency by identifying the source of the problem and addressing it.

The CCMC is a non-profit Christian ministry that strives to improve the quality of life of the needy in the Hot Springs area. It does so by providing general medical care to the uninsured, dental and vision services to the under-insured and serving as an advocate for the needy. CCMC collaborates with other organizations and their clients to provide seamless coordination of care of community services. As an act of Christian kindness, CCMC strives to improve physical, spiritual and social well-being while encouraging responsible lifestyles.

Description

CCMC was formed in 1996 with the first date of services 1/28/1997. Since that time we have provided medical services to more than 11,000 unique patients. CCMC provides more than $1 million in medical services to its patient base each year. We operate with a primarily volunteer staff, from physicians, pharmacists to office/clerical help. After the implementation of the Affordable Care Act of 2014, CCMC expanded services to include a long term solution for individuals living at or near poverty. We partnered with the Bridges Out of Poverty communities to provide this unique research based approach to poverty at an individual and community level.
