Redeemer Ministry Corps

The Community Health Worker position provides support and connects patients, families and community members with educational and community resources to ensure well-being and quality of life.

  • Collaborates with members of the patient’s care team, including: clinical social workers behavioral health specialists, nurses and physicians
  • Educating patients and families in the home or community site
  • Link patients with health services and community resources - housing, legal, insurance, etc
  • Provide emotional support and encouragement
  • Assist with navigating the health care and human service systems
  • Document encounters in EMR databases
  • Visit with patients in the home setting who are not eligible for VNA services
  • Assess patient safety within the home, and report any concerns to the home physician
  • Perform vitals
  • Assist physicians in conducting Wellness/Health Assessment Screenings
  • Assist patient with doctor’s office visits as needed
  • Work with CC or PHG to assist in closing gaps in care
  • Utilize chronic condition checklist tool as appropriate for the needs of the assigned patient
Optional - assist patients in need with transportation and accompany to medical appointments

This opportunity is part of Redeemer Ministry Corps (RMC), a long-term (11 month) volunteer program located in the Greater Philadelphia area.

RMC volunteers are called to bring a caring, comforting and healing presence to those in need through service in healthcare and social services. Volunteers live together in community and serve full-time in ministries of the Sisters of the Redeemer.

Mission Statement

Redeemer Ministry Corps is a long-term volunteer program sponsored by the Sisters of the Redeemer and offers volunteers the opportunity to journey beside those who suffer in some way, whether materially, spiritually or physically. The main components of the program are service, faith and community and are integrated throughout the year of service.

Description

Redeemer Ministry Corps welcomes women and men, 21 years of age and older who are committed to giving a year of service, living in community and participating in a simple lifestyle and evenings of prayer and reflection. RMC volunteers serve full time in ministries sponsored by the Sisters and join the Sisters once a week for community dinner and prayer. RMC volunteers are committed to serving vulnerable populations through service in healthcare, social service, elder care and environmental care. Ministry placements are tailored to fit the skills and gifts of the volunteers, as well as to meet the needs of the sites in which they serve.

We are looking for 5 volunteers to start the end of August 2019. Room and Board, health insurance, transportation and a monthly stipend provided. Check out our website for more information on service placements and our application process. We look forward to hearing from you!
