Advanced Care Planning is an essential component of guiding the medical community in the health care decision making process. The Health Care Proxy allows patients to appoint someone to make health care decisions on their behalf if they lose the ability to make decisions themselves.
Trained volunteers will be able to provide education to patients/families on the importance of the Health Care Proxy, and how to complete a Proxy form.
Qualified candidates must be at least 18 years of age, mature, compassionate individuals who maintain good communication and listening skills. Must be able to speak in front of groups as well as individually. Volunteers must be able to attend all trainings needed to participate in this program.
