POSITION SUMMARY:

The Patient/Family Services Volunteer provides various services as needed or requested to patients and their families including supportive care, comfort, and caregiver relief.

REPORTS TO: Volunteer Coordinator

SUPERVISES: None

QUALIFICATIONS:

Education: Completion of hospice volunteer training program.

Credentials: None

Experience: None

Core Competencies: Ability to deal with death and dying comfortably and demonstrate good listening skills. Willing to participate in on-going programs and peer support groups. Make a time commitment of one year to serve a minimum of two hours a week as needed.

Other: Valid driver’s license and auto insurance

FUNCTIONS & RESPONSIBILITIES:

  1. Initiates and maintains contact with assigned patient/family.

  1. Offers various forms of assistance, comfort measure, supportive services, including reading to and writing for patient, sitting at bedside, especially during dying process and caregiver relief for families.

  1. Assists patients and families to cope with problems of terminal illness and death; helps them to express feelings and concerns.

  1. Completes written reports of contact and submits to the Volunteer Coordinator.

  1. Refers to the Volunteer Coordinator any immediate needs, problems or concerns.

  1. Attends on-going education programs and peer support groups, as requested.

  1. Continues to provide emotional support to family during bereavement as appropriate or recommend bereavement follow-up by others.

PHYSICAL REQUIREMENTS

  1. Evidence of good physical and mental health.
  2. Able to lift 40 lbs. from floor to waist and waist to shoulder.
  3. Able to lift 25 lbs. from shoulder to overhead.
  4. Able to perform patient transfers, utilizing appropriate techniques, up to 150 lbs
  5. Average vision/hearing
  6. Frequent bending and reaching
  7. Repetitive standing, walking and use of hands.
  8. Phone communication skills.
  9. Able to drive a car to patient homes 2 to 4 hours per day.

Protective Equipment Required = As required by task and procedure

Exposure/Risk Category = II - Exposure to blood, blood products and body fluids not likely but may occur

HIPAA Level = 2
