The Long-Term Follow-Up Initiative (LTF) seeks to assess the quality of life, major life events and the overall long-term impact of JRI services after clients leave our programs. The LTF initiative reaches out to clients both during and after discharge, and involves completion of several surveys in the months and years following care. Our hope is to better understand the strengths and limitations of our services so that we can continue to improve our programming for the individuals we serve. We also hope that the LTF initiative will allow us to maintain a connection with clients and hopefully ease their transition into life post-treatment, with access to all of JRI’s services at their fingertips as needed for the individuals we serve. Volunteers will assist with the following tasks:

• Scanning and saving forms
• Filing
• Printing/copying surveys
• Preparing and mailing survey packets
• Data entry
• Updating / maintaining client contact info
• Updating spreadsheets
• Forwarding (copy & paste template) emails to collect basic information
• Assisting in scheduling meetings and calls
