Mission Statement
Ace Home Health and Hospice, Inc., is CHAP certified (Community Health Accreditation Partner) and provides comprehensive, patient-centered home health care services and compassionate end-of-life care to terminally ill patients all individualized to meet Patient’s needs as coordinated by each patient’s physician.
Description
Our vision is that our community, our patients, and their families have access to outstanding care and bereavement support and education to deal with end of life issues and challenges. The Interdisciplinary Hospice care team includes Physicians, Nurses, Home Health Aids, Chaplains (religious and spiritual counselors), Social Workers and Volunteers who provide a range of medical and support services. Ace Hospice Services and staff are culturally conscious as well as clinically skilled and compassionately first-rate. This combination promotes a supported, considerate and holistic approach to the end of life. We support our patients and their families in finding relief from pain, quality to the time they have remaining, and personal fulfillment as they deal with end-of-life concerns. We make every effort to support people in transition, sadness and loss while fostering dignity, connection, enjoyment and peace.
