Community Health Services Volunteer
JOB SUMMARY:

Performs diversified work of moderate complexity to promote and market Foothill Community Health Clinic (FCHC) services through community outreach, assisting patients with enrollment into our programs and services (i.e. Student Health & Wellness, Produce Distribution, Health Coverage), and support our community health screening and health education events that include medical, dental, vision and behavioral health services for the underserved population.

DUTIES AND RESPONSIBILITIES MAY INCLUDE BUT NOT LIMITED TO:

• Participate and assist with public health events at local schools, churches, businesses, and other public settings.
• Document and record all patient information into FCHC’s electronic health record system (ECW) under the guidance and supervision of an FCHC staff.
• Perform verification of patient information such as insurance coverage, contact information, appointment schedules, and other information related to our patient process.
• Interact with patients and health consumers via by phone or in-person to schedule appointment and/ or phone call reminders to confirm patient’s participation.
• May provide translation support for our patients and health consumers as needed
• Conduct general office duties that include filing, organizing, and edit new patient documents, assist in creating marketing materials, and greet our patients and health consumers.
• May be assigned to assist in our monthly produce distribution in pre-packaging produce, outreach to community about FCHC services, and assist with logistical preparations.

QUALIFICATIONS:

1. Ability to work independently, organized, strong communication and interpersonal skills, interest in health education, bilingual preferred but not required, task oriented, and able to work on a team, and flexible work schedule.
