Are you looking to gain experience in social services? Are you interested in gaining experience in Development? Would you like to help our clients achieve their health goals? We are looking for a dedicated volunteer to help in our Health Home Program.
The Full Life Care Health Home Program is a person-centered and holistic-minded care coordination service. Full Life Care is contracted with the Health Care Authority; Washington State’s Medicaid agency; to facilitate and provide Health Home services. The Health Home Program encompasses six fundamental services including comprehensive care management, care coordination, transitional care support, referral navigation, individual and family support, and health education and promotion. Medicaid beneficiaries are eligible for Health Home services if assessed to be at high risk of hospitalization in the next year and have one chronic condition and are at risk of another. Health Home-eligible Medicaid beneficiaries are engaged in services by completing a Health Action Plan with the Health Home Care Coordinator. Health Home Care Coordinators meet in-person at beneficiary’s location of choice on a monthly basis while coaching and collaborating with beneficiary on HAP-depicted action steps to improve their health and overall quality of life.
We are looking for a volunteer to help with the following:
- Coordinate and manage client donation requests from Care Coordinators
- Send emails, letters, or cold-call organizations in order to solicit in-kind donations
- Pick up and deliver and/or coordinate delivery of in-kind donations
- Keep track of in-kind donations and provide documentation to share with Development Department
- Attend Care Coordinator meetings in client homes; take photos, gather quotes, and write up summary of how donations have helped clients meet their health action plan goals
- Stories will be shared with donors and/or in Full Life Care publications or social media
Requirements:
- Experience or interest in Social Services
- Tech savvy (should be familiar with Excel, Microsoft Outlook, Teams, etc.
Preferred Qualifications:
- Car and/or way to get around in order to deliver in-kind donations
- Some knowledge of Seattle resources/organizations
- Great communication skills; comfortable speaking on behalf of Full Life Care
- Interest in learning, exploring and conducting research
- Thoughtful, creative, and very organized
Time Commitment:
- Volunteer should be able to commit to a minimum of 3 hours per week for at least 3 months; we would prefer a 3 - 6 month commitment
Location:
- Volunteer can choose to work from one of Full Life's offices in First Hill or South Seattle; option to work remotely and communicate via email and phone
