The Wellness Coordinator/Coach at the Chippewa Valley is a part-time position, averaging 8 hours per week. Must be able to work Tuesday evenings and Thursday daytime hours. Salary based on experience.
Lead the "Healthier You" Wellness Program of the Chippewa Valley Free Clinic. This leadership includes
- recruiting and enrolling patients into the program,
- collaboratively establishing individual patient goals,
- providing education, motivation, and support for all patients in the program,
- partnering with Clinic staff and volunteers, as well as community agencies,
- documenting patient and program outcomes, and
- serving as the spokesperson for the program within the Clinic and the wider Chippewa Valley community when requested by supervisor
- Health-related baccalaureate or associate degree from an accredited institution (e.g., nutrition, nursing, chronic disease management, health education)
- Wisconsin licensure and/or professional credentials, as applicable

Our Mission is provide health care to and advocacy for the people of the Chippewa Valley who have no health care alternative.
Description
We provide free medical services to the poor and uninsured in the Chippewa Valley. We host a general walk-in clinic every Tuesday night as well as a bi-monthly Diabetic Clinic, bi-monthly Mental Health Clinic and monthly Vision Clinic.
Through the hard work of dedicated volunteers we provide medical examinations, patient advocacy, lab services, and free medications within limits.
We are located at 1030 Oakridge Dr in Eau Claire, WI.
