Service Area Map
Our service area includes 10 counties: Lake, Cook, McHenry, Kane, Boone, Will, DuPage, Winnebago, Kendall, & DeKalb. The majority of our care is provided in patients' homes throughout these counties, but we also offer 5 Hospice Centers (Arlington Heights, Barrington, Chicago, Glenview, Woodstock) and office locations in Lombard, Olympia Fields and Rockford.

Volunteer in your community and set your own schedule! JourneyCare's Care Companion volunteers support hospice patients and families in their home, which may be a private residence or senior living community.

Who: JourneyCare serves 3000 patients each day and has the largest pediatric program in Illinois.
What: A Care Companion is simply a friend who visits (on a routine or occasional basis) with a child or adult receiving hospice care. They provide companionship, emotional support, and, above all, offer a presence, an ear to listen to stories and an understanding smile. You do not provide hands on medical care.
When: Work with the patient and family to determine your schedule. Care Companions often see their patient once a week or once every other week. Visits for socialization can be as short as 30 minutes and a respite visit can last no longer than 4 hours.
Where: You will visit patients in their private residence or senior living community.
Why: Care Companions also provide respite support so tired, stressed caregivers can take a much-needed break. Volunteers can support pediatric families by doing some light housekeeping or spending time with the patient's siblings.
How to get started: Fill out the JourneyCare Volunteer Application and register for an orientation: http://journeycare.org/volunteer/apply-volunteer/

We will cover the following: Hospice 101, Overview of JourneyCare, Our Services & How You Can Help (we offer 50+ volunteer roles!), Communication, Boundaries & Confidentiality and Documentation. You will also have an opportunity to interact with one or more of JourneyCare's current volunteers. We also require some online courses, which will help you to feel more prepared for this role.

Mission Statement

Enriching lives through expert and compassionate care. JourneyCare's Vision: Transforming lives by building a community where exceptional palliative, supportive and end - of- life care is accepted, expected and available to all.

Description

JourneyCare's Vision:

Transforming lives by building a community where exceptional palliative, supportive and end-of-life care is accepted, expected and available to all.

We provide pain and symptom management and end-of-life expertise to residents of a 13-county service area in northern Illinois. Offering both in-home care and five Hospice CareCenters, we are known for our broad continuum of care and innovative programming.

Beyond traditional services and a comprehensive bereavement program, we have the largest pediatric hospice and palliative care program in Illinois and we embrace non-traditional therapies as well as open access for patients receiving aggressive treatment.

Our mission is achieved by a team of board-certified physicians and highly-skilled nurses, therapists, counselors, social workers, chaplains, and volunteers.

JourneyCare has never turned anyone away due to inability to pay because our promise is to meet the patient where they are, build trust, and share the journey, regardless of reimbursement rates. We seek to put the patient in control of their care--because it's the patient's experience and the family's memory.

We are here to help when patients and families need it most. We begin by listening and answering the questions of those who come to us. We know that each experience is personal--and so is our care. We offer the physical, emotional, and spiritual support that's needed to welcome each new day.
