Mission Statement

Evergreen is dedicated to providing a platform for people, age 50 and beyond, to have the opportunity to reach their maximum potential in living life to the fullest and contribute to a thriving community.

Description

Care Services provides a variety of community resources such as: Home Delivered Meals, In-Home Care, The Day Center, a Bathing Program, Caregiver Training and Support, Medication Set-UP and a Resource Office. Like our other branch, the Commons of Evergreen, two of our programs, The Day Center and Home Delivered Meals, rely on volunteers to succeed. The Day Center of Evergreen offers outstanding assisted care for adults each weekday. We are a leader in community-based care for individuals with Alzheimer's disease and other forms of dementia. Participants can engage in meaningful and therapeutic activities which help meet their social, emotional, spiritual, creative and physical needs while their caregivers get some much deserved respite. Home Delivered Meals offer a tasty, nutritious meal delivered around the noon hour plus a wellness check by caring volunteer drivers
