As a Case Manager, you will be trained to provide supportive resource information to those experiencing homelessness to help lead them on a path to permanent housing. Helping people break barriers and reach milestones, you are walking alongside them in their journey to become self-sufficient and affirm their dignity.
With new clients, you will go through an intake form. If they are continuing clients, case notes are added to their files. Every client signs in to track foot traffic. With existing clients, we go over what we discussed at our last visit and find out what has worked and what hasn’t. The relationship building is an important part of the process as we gain their trust and advocate on their behalf. In this process, it will give them hope & confidence to know that they are not alone in this journey and that someone cares for their well-being. We also record Milestones that have been completed which is a joy for both the client and volunteer!
We run the Case Management Program during our office hours during the week so we need someone available Mondays - Wednesdays in the time frame of 1pm-5pm. This will depend on your availability so any help is appreciated. We use Google Drive and the 211 Database to record files and update our resource information.
We are open to volunteers who can be creative and hands-on with projects that support our homeless friends and partners! Volunteers have the opportunity to create and plan events to not only inform the public of this service to the homeless community, but to keep our hygiene inventory stocked and receive monetary donations that will support our clients and the program.

We understand that people who are experiencing a crisis can do better for themselves when they know they have someone who cares and looks to understand them while being connected to supportive resources that enhances their livelihood. As we walk with them in their journey, we help lead them to self-sufficiency and housing solutions.
Description
Our Case Manager Volunteer do an intake with clients to gain the proper information to assess their causes of homelessness, find solutions for their basic needs while connecting them to resources and services that lead to earned income and housing.
Our goal is to get people from Point A of where they are now to Point B of reaching their housing goals, walking with them in their journey in between. We value building relationships and an understanding with those we assist.
