We are looking for a variety of consistent community members, students, interpreters, and healthcare professionals to volunteer at our Monday evening free clinics. Several different opportunities and times are available. Community members and students will have the opportunity to choose which volunteer position they would like to assist with including: greeting, scanning charts, medical/ dental registration, room filling, clinic set-up, and more! All of our volunteer opportunities are listed on our website. There are also many opportunities for physicians, nurses, dentists, hygienists, and other healthcare professionals. Mission Statement
Physicians CareConnection is a volunteer-based charitable organization providing patient centered care coordination for people who are vulnerable (ANY person who has healthcare challenges due to individual barriers) in Central Ohio. Our organization was established over 20 years ago by the Columbus Medical Association to enable health care professionals and volunteers to provide quality healthcare for those most vulnerable in the community.
Description
Poverty, unemployment, poor nutrition, substandard housing, limited educational opportunities, and chronic stress are all barriers to health and well-being of people who are vulnerable. Using our unique model of patient centered care coordination; PCC has achieved successful healthcare outcomes for high risk, hard to reach populations in Central Ohio. Our model supports medical professionals operating at the top of their licenses to focus on the treatment and diagnosing of disease. PCC acts as an extension of the medical team assessing and addressing social, behavioral and "place-based" risk factors that negatively impact healthcare outcomes. Patients who have received our services have realized improved physical and mental health status, easier access to care, and a reduction in their use of emergency room services. We provide primary care, dental care, prenatal care, specialized services, and more!
