The Diabetes Foundation assists children, teens and adults living with type 1, type 2, gestational and prediabetes to overcome the associated financial, physical and emotional challenges. Our Diabetes Resource Advocate Program helps individuals with diabetes (whom the Diabetes Foundation calls participants) connect with local resources to manage their health and follow the care plan set forth by their doctor. This includes creating a healthcare team; tips and tools for disease management; nutrition, fitness and wellness programs; educational offerings provided by hospitals and community organizations; age-appropriate support groups; and short- and long-term prescription assistance programs. Comprehensive training is provided to ensure the effectiveness of volunteers and the program.
REPORTS TO: Program Director, Program Manager
RESPONSIBILITIES:
- Support a portfolio of regular participants
- Field incoming calls as needed
- Research as need to update resources/find new support
- Program support as needed
- Work one day/week, 4-6 hours/day, between 9 a.m. and 5 p.m.
- Due to the personal interaction with participants, a one-year commitment to the Diabetes Resource Advocate Program is preferred
SKILLS/QUALIFICATIONS:
- Empathetic, energetic personality
- Experience interacting with patients/customers via telephone and/or in person
- Familiarity with diabetes - either through personal experience or medical training -helpful, but not required
- Bilingual English/Spanish proficiency a plus
