Providence ElderPlace is an innovative program of services for older adults. Providence ElderPlace helps older adults remain as healthy and independent as possible in the community for as long as possible.
In Home Friendly Visitors: The ElderPlace In Home Friendly Visitor program provides one-on-one support to our participants within their living environment outside of the ElderPlace facilities. ElderPlace participants live throughout King County.

PACE programs keep older adults as healthy as possible in the community by providing comprehensive health care and social services including: primary and specialty medical care, a day health program, social work services, rehabilitation, housing (if necessary) and much more.
Description
Providence ElderPlace is an innovative program of health care and social services for older adults. Our model of care is known as PACE (Program of All Inclusive Care for the Elderly).Participants attend the Providence ElderPlace Center on a regular basis, and transportation is provided. The ElderPlace team of health care and social service professionals and our affiliates provide comprehensive integrated care to our participants.
