LifeLong Trust Health Center provides primary care medical services and comprehensive behavioral health care to adults with disabilities and histories of homelessness. The clinic serves as a resource center to empower patients to be independent and in control of their wellbeing. The staff and partner organizations provide a multitude of services such as medical and behavioral health care, health coach support, and wellness groups. We promote clients’ overall health, and as a volunteer, you will be an integral part of a multi-disciplinary health care team.

LifeLong Trust is seeking a volunteer who are interested in working with underserved patients in an integrated primary care setting. Volunteer duties center around coordinating services for patients, with particular emphasis on housing resources.

Duties include:

  • Researching affordable housing resources in Alameda and Contra Costa Counties.
  • Connecting patients to housing opportunities.
  • Preparing & facilitating wellness groups.
  • Providing direct patient services
  • Assisting with administrative work when needed.
  • Coordinate services with Health Coaches, Housing Coordinator and AmeriCorps member.

Depending on the level of interest, volunteers have the flexibility to design their own health education initiatives and health outreach projects. The role is versatile and can be adapted to suit the interest and goals of the individual. Volunteers will also become proficient in using Lifelong’s electronic health records system (NextGen) and have the opportunity to shadow a full range of medical and mental providers.

Previous volunteers have gone on to graduate schools in the health professions, including public health, Nurse Practitioner and medical schools. We are looking for individuals who are willing to learn, able to work independently, and eager to be part of a team.

Hours: We ask that volunteers be able to commit a minimum 6 hours a week (Monday-Friday) for at least 6 consecutive months.
