Hospice Community Care ’s hospice interdisciplinary team offers superior service to patients by providing year-round 24-hour professional support. This experienced group of professionals consists of a diverse group of professionals, each with their own specialized focus. Our hospice care team will work closely and collaboratively with patients and other team members to ensure that the patient receives the most effective pain management and symptom relief while also nurturing their physical, emotional, spiritual, and social needs.
