Description
Neighborhood HealthSource is a federally qualified health center, operating four primary care clinics in the neighborhoods of North and Northeast Minneapolis and in Coon Rapids for 49 years. Our mission is to improve the health of our communities by providing quality health care services that are affordable and accessible. Clinic services include basic and preventive health care for adults and children, screenings and immunizations, minor procedures, chronic disease management, mental health/substance use disorder counseling and maternity and newborn care. NHS also provides various community outreach programs, focused on childhood obesity, stroke and diabetes, family planning and STDs. NHS provides medical care to patients at tens of thousands of visits a year, including a half-million dollars of subsidized care to individuals in need.
