1. Assignment Title
Bereavement Services Volunteer at the Charlie Norwood VA Medical Center

2. Purpose of Assignment
To provide supportive services to help meet patient/family emotional needs in bereavement follow-up
3. Tasks
· Provide telephone contacts per VA policy
· Provide sympathy cards for families
· Attend Hospice Veteran’s Partnership meeting quarterly
· Provide service report of calls and mailings to Hospice/Palliative Care Coordinator
· Provide educational materials as needed
4. Time Commitment
4 hours per week
5. Requirements
· Pass a background check
· . Demonstrates experience in the areas of life-threatening and chronic illness, grief and loss.
· Skill in both oral and written communication
6. Training and Skill Development
· Initial and annual volunteer training.
· Assignment specific training
7. Supervision
Volunteer reports to: Linda Doyle, RN Hospice/Palliative Care Coordinator
Feedback: Annual written

The Charlie Norwood VA Medical Center is a two division medical center located in Augusta. We provide primary care, acute care, and long term care planning for our Veteran patients. Voluntary Service is all about volunteering, donations, and creating opportunities for all of us to give back and care for our Veterans.
