Health Self-Management Leaders facilitate evidence-based programs that focus on improving the lives of older adults across Los Angeles county. Volunteers have opportunities to lead one or more of the following programs:
- Healthier Living (Chronic Disease Self-Management Program): A small group program for people living with chronic conditions
- A Matter of Balance: A falls prevention program for adults who have fallen or have a fear of falling
- Arthritis Foundation Exercise Program: An exercise class for adults with arthritis or other similar joint conditions
Programs last 6-8 weeks with 1-2 sessions per week. Weekly time commitment is 4-6 hours after training. Training commitment varies by program from 10 hours online to 4 days in person.
Abilities and Requirements:
- Comfort presenting for, and encouraging discussions in groups
- Access to reliable transportation
- Availability between 9:00 AM - 5:00 PM on weekdays
- Commitment to leading at least three workshops per year and attending training
- Spanish speakers are especially encouraged to apply!
Opportunities are available throughout LA County, but the following areas are high priority:
- City of LA
- South Bay
- East San Gabriel Valley / Pomona Valley
- Antelope Valley

The Health Self-Management Services Department works to design and implement new models of evidence-based care that address the key social determinants of health.
Description
We are seeking committed volunteers to serve as Health Self-Management Leaders in LA County. Volunteers facilitate one or more evidence-based programs designed to improve the lives of older adults such as Healthier Living, A Matter of Balance, and the Arthritis Foundation Exercise Program. Volunteers are asked to commit a few hours per week during weekdays and to facilitate at least three programs per year, and are provided with leader training and skill development.
