We are starting a new volunteer Patient Navigator position at our Davis Community Clinic! The volunteer Patient Navigator will be located in our front lobby area to help welcome each patient to CommuniCare, direct patients to the department they are looking for, assist them in filling out clinic forms, assist patients in enrolling into their patient portal account, and answer any questions that patients have.
Requirements
- Bilingual in English and Spanish
- Able to stand for long periods of time
- Available a minimum of two 4-hour shifts per week (shifts are Monday-Friday 8-12 and 1-5)
- Very friendly and outgoing and willing to approach people to offer assistance
- Professional and courteous
- Able to use a tablet to navigate websites, email logins, and electronic medical record provider schedules

Description
Established in 2023, CommuniCare+OLE is a union of two health centers with deep roots in their respective communities with reputations for providing high-quality primary care to patients and clients.: OLE Health of Napa and Solano Counties, and CommuniCare Health Centers of Yolo County.
Building on a legacy established by each organization in 1972, CommuniCare+OLE is a network of federally qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.
In October 2023, OLE Health of Napa and Solano and CommuniCare Health Centers of Yolo County announced a legally executed merger between the two organizations and unveiled their new brand: CommuniCare OLE. As a critical part of the healthcare safety net, CommuniCare OLE ensures care for everyone, regardless of ability to pay or insurance status.
CommuniCare+OLE’s services include Primary Care, Behavioral Health, Substance Use Services, Perinatal Care, Dental Care, Pediatrics, Sexual Health, Women’s Health, LGBTQIA+ Affirming Care, Diabetes Education and Food and Nutrition services.
